Please review the following questions. If you cannot answer "yes" to all of them at any time during your hospital stay or you have medical or safety concerns, please ask to speak to the clinical manager on your nursing unit right away.

The Universal Medication Form is a tool developed for consumers to keep track of all of prescription medications, over-the-counter medications, and herbal supplements a person takes at home. You can get these forms from many places such as hospitals, physician offices, pharmacies and health fairs. Take the form with you and give copies to each of your doctors, your pharmacist and your dentist. Have them help you update the form if your medications change.
To keep track of your medications online, sign up for MyProvidence, a free and confidential health management tool.
