Every program begins with the Personal Wellness Profile—a scientifically developed questionnaire that is used to describe an individual’s level of health risk. Questions take into account lifestyle and health factors such as nutrition practices, stress, biometrics and other measurements of health risks. After completion, the Personal Wellness Profile is assessed and scored.
Providence Hospitals’ Corporate Health & Wellness Program uses a paper-and-pencil or online questionnaire that is administered on-site by our trained employees. Because a third-party administers the Personal Wellness Profile, your employees can be assured confidentiality of each individual’s answers and scores.
While the Personal Wellness Profile provides self-reported data on employee health risks, biometric screenings are objective measurements of threats to current health. Our employees come to your work site to measure height, weight and blood pressure. Results of testing from a simple finger stick blood sample or vena puncture blood draw will measure blood glucose levels, total cholesterol (with separate HDL and LDL levels), very low-density lipoproteins (VLDL) and triglycerides.
Our staff is available to discuss other screenings with you that may benefit your employee population.
After the questionnaire and biometric screening are complete, participants will receive a one on-one consultation with a member of our professional team to review results and discuss any immediate health risks. Participants with test results outside of normal ranges will be referred for a physician follow-up appointment.
A group report detailing your company’s aggregate population risks will be created from the results of the Personal Wellness Profile and biometric screenings. In consultation with Providence Health & Wellness professionals, this report will inform your decisions about the health and wellness education programs you can offer to lower your company’s top health risks.
