Admission Information

Nurse behind deskWe want your admission and registration process to go as smoothly as possible. We have created the following guide to help ease the process.

Please bring the following items with you at the time of registration:

  • Active insurance cards
  • Picture ID

If you are being admitted, please leave all of your valuables at home for safekeeping.

Upon admission, you will be required to sign the following forms and authorizations (if you are over 18 years of age):

  • Consent and Conditions of Treatment
  • Privacy Practice Notification
  • Other payer and payment-related forms

Depending on your insurance carrier, you may be required to sign additional forms.

We kindly request that all co-pays, deductibles and out-of-pocket money due is paid at the time of admission for elective procedures. If you are unable to pay your portion of the bill, please let the associate know so that other options such as a payment plan or financial assistance may be discussed.

If you know your impending date of admission, you are encouraged to pre-admit. Preadmission will allow for some of the paperwork to be processed prior to your actual date of admission.

Please be sure to check with your health insurance provider for any specific authorizations or pre-certifications that may be necessary before admission. Often, if these incidentals are not handled up front, a financial penalty to the patient can result. Because there are several insurance companies, all with their own specific requirements, it is wise to check this information on your own. The hospital staff may not be familiar with your carrier and its requirements.

The Admitting Office for Providence Orthopaedic & Neuro Spine Institute is located just off to the left as you enter the main hospital lobby and is open from 5:45 a.m. until 4:30 p.m. After 4:40 p.m., the Emergency department registration associates handle all admissions and registrations. There is always someone to assist you 24 hours a day, seven days a week. Should you have any questions regarding the admission or registration process, please call us at (803) 856-4970. Please note that your bill does not include physician fees such as those from a surgeon, anesthesiologist, radiologist or pathologist. Diagnostic and therapeutic services provided by physician and physician groups are billed separately. These bills will come from the physician, not from Providence.
